I am tring to design a static page that has an add to cart button with qty and options on the page. Please visit the example site I am tring to emulate

We have quick order forms just like your example. We started out coding a static page but then discovered we could get the same results dynamically. I can't remember all the details of setting it up but here are the settings that show for the quick order form category pages we have:

Parent Category = Quick Order Forms
Category Name = Product Group 1
Category Visible says (Parent Category Visible will be used.)
Category Description =
<div align="center"><br><span class="cat-head">Product Group 1<br><span class="green-ws">Quick Order Form</span></span><br><br>
To add items to your wholesale basket, specify quantities and click the <b>Add</b> checkboxes<br> to the right of each item, then click the <b>Add To Cart</b> button at the bottom of the form.</div>

Category Description Below Products = <div align="right" class="darkred">
*** Be sure that you have clicked the <b>Add</b> checkbox to the right of<br>each item you are ordering before clicking the <b>Add To Cart</b> button.</div><br><br>
Subcategory Display Columns = 3
Category Graphic Placement = Below
Product Display Mode = Checkboxes
Default Sort By = Title
Display Rows = 50
Display Featured Products Only = Y
List Price Show It = N
Description Short Show It = N

ALL other fields are blank.

Don't forget to add the add to cart button coding in the Category Description Below Products field. Not shown in screen shot.

The small large and refill are options on the product page but each have their own product code for inventory control so we just put all the options in this category and then they each show in the grid.

Thank you very much, unfortunately this is for a client that is very specific and needs an actual ad to cart button next to each item. The add check box is not a clean enough look for the client. Plus I need the options for colors on the page. I am sure this can be done with Java Script I am not sure how. Anyone know?
